The new school year is right around the corner and the State of Illinois requires that all children entering preschool, kindergarten, sixth grade and ninth grade have a complete physical exam and certain immunizations. Please make your appointments now by calling us at 630.897.6044.
Due to high volumes please call to schedule your child’s appointment soon. We ask that you call well in advance of the start of the school year to schedule your appointment -this can be done as early as springtime.
The State of Illinois requires that all children entering preschool, kindergarten, sixth grade and ninth grade have a complete physical exam and certain immunizations. This exam is valid for one year. The school will mail you a form in the spring before that school year. Please make sure the top of the form is filled out completely.
The State of Illinois also requires a pre-participation physical exam prior to playing any middle school or high school sport. This exam is also only valid for only one year.
