Your Child's Visit

6-7 years

All patients will need to complete the following forms prior to your child’s appointment:

• Pediatric Health Form (if not already completed please complete)
• Lead screening (6yrs not 7yrs)
• TB screening
• Hyperlipidemia screening
• 6 years Pediatric Well Visit Questionnaire
• 7 years Pediatric Well Visit Questionnaire
